You can apply to the Graduate School by downloading the form using the link below and mailing it to the Graduate School at Eastern Kentucky University. Once the form has been downloaded, you can enter all of the information into the appropriate fields on the form. When you have completed the form, print it out and mail it to the Graduate School. Please note that any information typed into the form is not saved. You should also print a copy of the form for your records.
